ACCESS TO RECORDS
You have a right to access your own medical record and we can arrange a mutually convenient
time for you to come in to view it. You have no right to view your record immediately but to view
your record within a reasonable timeframe.
Under some circumstances you may view the medical record of another member of family but this is only with their expressed consent in writing.
There is also a notional fee for accessing your medical record of £10 and a further fee if you wish to have a copy of any part of your record.

photo identification or collect a form from the reception team when you are passing.
